Incident Reporting

Notice Requirement: All incidents involving Earthwise personnel, equipment, or property damage must be reported in writing within twenty-four (24) hours. Failure to meet this deadline may result in the waiver of claims. Submission of this report does not constitute admission of liability by Earthwise Enterprise Inc. All claims are subject to investigation and the limitations of liability.

1. Mandatory Reporting Steps

To file a valid report, the Customer must:

1. Secure the Site: Ensure no further damage occurs and that the area is safe.
2. Document the Scene: Take at least four (4) high-resolution photos of the equipment, the damage, and the surrounding area.
3. Identify Witnesses: Collect names and contact information for any individuals present at the time of the incident.
4. Submit the Form: Complete the “Official Incident Report” below.
